UnitedHealthcare offers the most extensive series of Special Needs Plans in the market with plans available in 34 states. Coverage is available through HMO and PPO plans in many states with plans for individuals with chronic health conditions or who have a limited income.
Plans for specific long-term illnesses are available to individuals who have one of the following chronic conditions: Asthma, Arthritis, Cardiovascular Disease, Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure, Dementia, Diabetes, or Hypertension. Plans include extra benefits not covered by Original Medicare such as Part D Prescription Drug coverage, no cost for certain labs, and access to products from a personal healthcare catalog.
Plans for individuals with limited incomes are available. If your client is a dual-eligible (enrolled in Medicare and Medicaid), Evercare may be a better choice for full health care coverage. The benefits add to what Medicaid offers so the client gets more coverage. With Evercare plans, your client receives coordination of all Medicare benefits, support of Care Managers, personal help connecting with Medicaid covered services, extra benefits not covered by Original Medicare and Medicaid like Part D Prescription Drug coverage and transportation to doctor appointments.
