In 2008 CIGNA Health Care of Arizona introduced the CIGNA Medicare Select Plus Rx- Chronic Care Plan. The plan is a great fit for Medicare beneficiaries with one or more of the following chronic conditions.
DiabetesHigh blood pressure
High Cholesterol/Chronic Heart Failure

Who is eligible
A Medicare beneficiary who has one or more of the qualifying conditions listed above and lives in the CIGNA Medicare Select service area (Maricopa County and the City of Apache Junction)
Even Medicare beneficiaries without Part D can enroll in the Chronic Care plan throughout the year
